Virtual Care Coordinator (Hybrid) Remote and On-Site Roles (locally) Akron-Canton Ohio Area The Virtual Care Coordinator (Hybrid) is a licensed professional, who works as a member of the provider's clinical staff, who supports the remote care management of patients receiving Remote Physiological Monitoring, Chronic Care Management, Remote Therapeutic Monitoring, Transitional Care Management and Behavioral Health Integration.
The Virtual Care Coordinator (Hybrid) serves assigned local medical practices as clinical staff for physician groups contracted with Vitality HealthCare services.
Reports to:
Senior Vice President of Administration Employment Structure:
1099 Experience Necessary to perform job duties:
? At least 3 years of experience as a clinical staff member professionally trained as a Medical Assistant ? Virtual Care experience is strongly preferred.
? Experience in using multiple Electronic Health Records for physician practices.
? system.
? Telehealth experience strongly preferred.
Skills necessary to perform job duties:
? Ability to communicate effectively with various physicians, physician's assistants, and nurse practitioners.
? Exceptional written and verbal communication skills ? Competent in using multiple electronic health records that support the suite of Lifetime Vitality Providers across the country.
? Proficient with Google suite/Microsoft office.
? Deep understanding of Social Determinants of Health and Health Literacy and the ability to modify auditory-verbal communication over the phone to meet the needs of our patients.
Daily job duties:
? Works on the consenting team, training team, care management and monitoring teams for virtual care ? Supports physicians for E/M telehealth visits.
? Follows protocols and scripts written by the nursing and physician teams.
? Speaks to patients and families over the phone or when in the practice-in person.
? Trains patients on how to use technology and take vital signs, following the physician and nursing protocols.
? Other duties as assigned and necessary for the success of the organization Physical Demands of Job:
? Sitting for long periods of time ? Requires a headset and a computer.
Structure/Schedule of Hours:
? Job is Hybrid- remote and in-clinic ? Weekends and Evenings may be required as assigned and needed for patient management and provider preference.
? Must maintain reliable internet service, throughout employment to meet the duties as described.
