The Community Health Worker (CHW) is a home visitor who is primarily focused on the client's health.
The CHW connects underserved clients to primary care, health insurance, and community resources so the client's health becomes the priority.
Minimum Requirements:
Must be over 18 years of age, have valid driver's license, and proof of insurance High School Diploma or General Education Diploma (GED) Must be able to travel all areas of Stark County and have reliable personal vehicle Must obtain CHW Certification within the first year of employment Ability to pass a federal and state background check and drug test Must maintain required caseload according to number of hours worked Must have knowledge of local resources and preferably, live in Stark County Key

Establish trusting relationships Conduct home-based interviews and enter data into computer Help patients set personal goals and attend primary care appointments as needed Connect clients to resources in their community (i.

) Exhibit excellent working relationships with patients and team members Ability to work with others of various ages, backgrounds, cultures, and life experiences Ability to maintain regular attendance on the job Job Type:
